Health Requirements & TCA Mobile Van
CPS requires updated health records for the school year:
- Physicals & Vaccines: Pre-K, Kindergarten, 6th Grade, and all new CPS students.
- Vision: Kindergarten and students new to Illinois.
- Dental: Kindergarten, 2nd Grade, and 6th Grade.
TCA Mobile Health Van is Coming!
Get your child’s exams done right at school on Thursday, August 13th (9:00 AM – 3:00 PM)!
- Sign-Up: Stop by the school this Wednesday or Thursday (10:00 AM – 2:00 PM) to reserve your slot.
- Paperwork: Pick up forms at the school during sign-up, or complete the attached consent forms ahead of time. (Forms can also be filled out on-site the day of the event).
